Physical activity is not suggested to patients because of bureaucracy



Newswise — HOUSTON – (June 19, 2012) – Lack of time, knowledge and training in health promotion and lack of success with changing patient behavior were among the top barriers to including effective physical activity counseling in the primary care setting, according to research by The University of Texas School of Public Health, part of The University of Texas Health Science Center at Houston (UTHealth).
“Individual and organization barriers must be addressed in order to incorporate counseling effectively,” said Emily Hebert, M.P.H., the study’s lead author and doctoral candidate at The University of Texas School of Public Health Austin Regional Campus, part of UTHealth. “Although these barriers may hinder a primary care provider to provide counseling, there are licensed counselors and health professionals who could work with the physicians to help patients achieve their goals.”
The article is published in the July issue of the British Journal of Sports Medicine.
Physical activity counseling is considered advice and discussion to encourage patients to increase and maintain physical activity.. The U.S. Department of Health and Human Services’ (HHS) Healthy People 2020 initiative recommends physicians provide intensive physical activity counseling and behavioral interventions to patients with chronic conditions such as cardiovascular disease and diabetes.
It is estimated that only 30 to 50 percent of U.S. physicians regularly provide counseling on physical activity to their patients, according to the article. Research has shown that physical activity reduces the risk of all-cause mortality, coronary heart disease, hypertension, type 2 diabetes, certain cancers and other chronic conditions.
“Evidence suggests that the most effective counseling includes more than just telling the patient they need to exercise or that exercise is important,” said Hebert. “Use of evidence-based counseling methods and strategies as well as following up with patients could help with decreasing a patient’s weight or reduce their chance of developing a chronic disease due to their lack of physical activity.”
The article also states providers are uncertain about the effectiveness of their counseling and feel they can only provide general advice about the importance of physical activity due to their lack of training in health promotion.
Recent studies suggest there may be some short-term benefits to counseling but the long-term impact remains uncertain, according to the article.
According to data from the Centers for Disease Control and Prevention’s National Health and Nutritional Examination Survey, less than 5 percent of U.S. adults meet the recommended amount of physical activity. The HHS recommends at least 150 minutes of physical activity a week.
Physical activity counseling has had varying levels of success in the primary care setting, according to the article. Hebert hopes the review will help future interventions address issues that potentially limit their effectiveness.
“Insufficient physical activity is an independent risk factor for numerous chronic diseases, such as obesity and type 2 diabetes and premature death,” said Kerem Shuval, Ph.D., assistant professor of epidemiology, human genetics and environmental sciences at the UT School of Public Health Dallas Regional Campus. “Primary care providers have the potential to play a key role in promoting physical activity among their patients; however numerous barriers hinder their ability to do so. This article summarizes barriers providers encounter during clinical practice and this systematic review may be utilized as a platform when designing physical activity interventions in primary care.” Shuval served as Hebert’s thesis adviser for the article.
Hebert reviewed 19 articles to summarize the perceptions and attitudes of primary care providers, as well as barriers to counseling. Physicians, primary care nurses and nurse practitioners were among the providers mentioned in the reviewed studies.
Lack of financial incentive/reimbursement for counseling was also listed as a barrier by primary care professionals in the study, which reviewed articles from 1991 to 2011. In November 2011, the Centers for Medicare & Medicaid Services announced that Medicare would add coverage for preventive services to reduce obesity, which includes counseling to promote weight loss through high intensity interventions on exercise and diet. Certain restrictions and guidelines must be followed in order for the primary care provider to receive reimbursement.
Providers also cited lack of training/knowledge of effective health promotion strategies as a barrier to their efforts. Hebert said incorporating health promotion counseling in medical school curriculum could help improve knowledge. “This could be incorporated through web-based courses, discussion of counseling in grand rounds or noon lectures, and mini-clinical exam evaluations.”
In addition, providers were more likely to provide counseling if the patient had cardiovascular disease symptoms, were obese or had hypertension or if the provider was active themselves.
Co-authors on the paper include Hebert, Shuval and Margaret Caughy, Sc.D., associate professor of health promotion and behavioral sciences at the UT School of Public Health Dallas Regional Campus.

The fighters were cheered on by a crowd of local fans as they took to the mats in the middle of “Praça da Estação” (Station Square).

Main event stars and UFC icons Wanderlei Silva and Rich Franklin were joined by the four The Ultimate Fighter Brasil Finalists – featherweights Godofredo Pepey and Rony Jason and middleweights Cezar Mutante and Sergio Moraes – as well as the co-main event fighters, heavyweight standouts Fabricio Werdum and Mike Russow.

Tai Chi increases brain size, benefits cognition in trial

Scientists from the University of South Florida and Fudan University in Shanghai found increases in brain volume and improvements on tests of memory and thinking in Chinese seniors who practiced Tai Chi three times a week, reports an article published today in the Journal of Alzheimer’s Disease.
Findings were based on an 8-month randomized controlled trial comparing those who practiced Tai Chi to a group who received no intervention. The same trial showed increases in brain volume and more limited cognitive improvements in a group that participated in lively discussions three times per week over the same time period.
Previous trials have shown increases in brain volume in people who participated in aerobic exercise, and in one of these trials, an improvement in memory was seen. However, this was the first trial to show that a less aerobic form of exercise, Tai Chi, as well as stimulating discussion led to similar increases in brain volume and improvements on psychological tests of memory and thinking.

The group that did not participate in the interventions showed brain shrinkage over the same time period, consistent with what generally has been observed for persons in their 60s and 70s.
Numerous studies have shown that dementia and the syndrome of gradual cognitive deterioration that precedes it is associated with increasing shrinkage of the brain as nerve cells and their connections are gradually lost.
“The ability to reverse this trend with physical exercise and increased mental activity implies that it may be possible to delay the onset of dementia in older persons through interventions that have many physical and mental health benefits,” said lead author Dr. James Mortimer, professor of epidemiology at the University of South Florida College of Public Health.
Research suggests that aerobic exercise is associated with increased production of brain growth factors. It remains to be determined whether forms of exercise like Tai Chi that include an important mental exercise component could lead to similar changes in the production of these factors. “If this is shown, then it would provide strong support to the concept of “use it or lose it” and encourage seniors to stay actively involved both intellectually and physically,” Dr. Mortimer said.
One question raised by the research is whether sustained physical and mental exercise can contribute to the prevention of Alzheimer’s disease, the most common dementing illness.
“Epidemiologic studies have shown repeatedly that individuals who engage in more physical exercise or are more socially active have a lower risk of Alzheimer’s disease,” Dr. Mortimer said. “The current findings suggest that this may be a result of growth and preservation of critical regions of the brain affected by this illness.”

A natural compound found in some fruits, nuts and red wine may enhance exercise training and performance, demonstrates newly published medical research from the University of Alberta.
Principal investigator Jason Dyck and his team found out in experiments that high doses of the natural compound resveratrol improved physical performance, heart function and muscle strength in lab models.
“We were excited when we saw that resveratrol showed results similar to what you would see from extensive endurance exercise training,” says Dyck, who works in the Faculty of Medicine & Dentistry as a researcher in the department of Pediatrics and the department of Pharmacology. “We immediately saw the potential for this and thought that we identified ‘improved exercise performance in a pill.’ ”
His team’s findings were published in the peer-reviewed Journal of Physiology in late May.
Dyck and his team will soon start starting testing resveratrol on diabetics with heart failure to see if the natural compound can improve heart function for this patient group. The 10-week study is expected to start within the next few months.
“I think resveratrol could help patient populations who want to exercise but are physically incapable. Resveratrol could mimic exercise for them or improve the benefits of the modest amount of exercise that they can do,” says Dyck. “It is very satisfying to progress from basic research in a lab to testing in people, in a short period of time.”

Cunningham A Free Agent, Eyes Move To Heavyweight


Former two time IBF Champion of the World, Steve ‘USS’ Cunningham, announced earlier today that he is now a promotional free agent, as well as stated that he is considering a move to the Heavyweight division.

Cunningham, whose second reign as Champion see him secure victories over Troy Ross and Enad Licina before controversially losing his crown, due to a dubious Technical Decision in favour of his opponent, Cuba’s Yoan Pablo Hernandez, in October 2011.

The IBF reacted instantly, ordering an immediate rematch, which took place on the 4th February, this time the bout went the distance, however the result went against the former Navy man.

Since the second Hernandez fight, Team Cunningham have been attempting to secure a meaningful Cruiserweight bout, preferably a title challenge, without any success.

On making the announcement Cunningham said, "I'm excited to be a free agent again and see what's out there. 

I feel like heavyweight is the right move for me right now, but I would still take the right cruiserweight fight.  

I would love to fight in America again, but I'll go wherever duty calls."

As many already now my love of combat sport is endless. From helping to fighters prepare for fights, to volunteering for amateur show, I am every where in the boxing and MMA community. I also have a love and passion for several charities close around the globe. 

I while back I was given the opportunity to come up with an idea. 
The idea has grown from something small to something huge. 
It can literally make a different to so many fighters as well as people recovering from injuries. 

I am currently in the process of coming up with a business plan to get the necessary funding to bring this project to life. Today, I took another step towards making the dream come true. 

I contest for a financial small business grant came across my desk. It has the potential to give me the necessary capital to get the project rolling.

Sao Paulo, Brazil - The first-ever middleweight winner of The Ultimate Fighter Brazil® will now be decided in a three round bout between Cezar Mutante and Serginho (Sergio) Moraes at UFC® 147: Silva vs. Franklin II, on June 23, at the Mineirinho Arena, in Belo Horizonte, Brazil. Moraes, a semifinalist of the first international edition of the UFC’s reality TV show, was chosen to replace of Daniel Sarafian, who was forced off the card due to an arm injury.

Moraes reached the TUF Brazil semifinals after beating Delson Heleno via submission on the ninth episode of the show. Originally a member of “Team Vitor”, he was moved to “Team Wanderlei” later in the season, squaring off against Daniel Sarafian for the first spot in the middleweight final. Though Sarafian won the bout, everyone was moved by Moraes story, a jiu-jitsu expert who overcame adversity in his personal life. It was this drive and success that made him the perfect choice to replace Sarafian. 

The middleweight contestants will square off in the fourth bout of the main card, ahead of a long awaited rematch between TUF Brazil coach Wanderlei Silva and former UFC middleweight champion Rich Franklin. The main card will also feature the TUF Brazil featherweight final, between Rony Jason and Godofredo Pepey, who along with Moraes and Mutante are competing for a lucrative contract with the UFC. 

Seven of the 11 bouts at UFC 147 will feature contestants from the first season of TUF Brazil. Due to the injury and the loss of his opponent, Renee Forte will be unable to fight and will make an appearance on a future UFC card. Episode 13 recap: Mutante KO’s his way into the finale and wins two bonuses

In the final episode of the first season of TUF Brazil, which aired Sunday, June 17, the last middleweight finalist was announced, as well as the season winners of best Fight, Knockout and Submission bonuses. The awards were announced by on-air personality Galvão Bueno. 

After knocking out Thiado Bodão, Cezar Mutante secured his place in the middleweight final and a spot on the UFC 147 card.

"What I did was focus on my training and what I was going to do out there. The opportunity presented itself at the right moment and I took it,” said Mutante, who now faces Serginho Moraes Saturday, June 23, at the Mineirinho Arena. 

The high kick that took Bodão down in the first round of their battle also assured Mutante of winning the Best Knockout of the season award. The Team Vitor fighter also took home the Raça Forte Fighter prize, given to the contestant who needed the least amount of time to win his fights during the show. 

The winner of the best Submission of the Season award was Rony Jason, for the armlock that gave him the win over Anistavio Gasparzinho. The best Fight of the Season award was given to Bodão and Massaranduba, who faced off in a two round battle that took Bodão into the semifinals. Each winner received $45,000 Brazilian Real.
